Life of a patient with ankylosing spondylitis

The bachelor thesis deals with the life of a patient with ankylosing spondylitis. The thesis is divided into a theoretical part and a practical part. The theoretical part presents anatomy and physiology of the spine, ankylosing spondylitis, its diagnosis and therapy. Furthermore, attention was paid to nursing care, communication and needs in everyday life of a patient with ankylosing spondylitis. The practical part of the bachelor's thesis was conducted through a qualitative research investigation, using the method of questioning, the technique of individual semi-structured interview. The study sample consisted of 7 patients with ankylosing spondylitis in a healthcare facility. Three objectives were chosen for the bachelor thesis. The first aim was to find out in which areas of normal daily activities ankylosing spondylitis affects the patient. The second aim was to investigate the role of the patient's family in the integration of the patient into normal daily activities. The third aim was to explore patients' perceptions of nurses' support in relation to their illness. In line with the objectives, research questions were set. Based on the results, it was found that patients with AS manage normal daily activities independently or with only minor assistance. Patients use compensatory aids to help them cope better with everyday activities. Treatment has a big impact on the patient's life, even though they use spa treatments and exercise daily. The research also showed that the support of family and friends plays a big role in the patient's life. Patients require more information from the nurse in collaboration with the doctor about treatment options, compensatory aids, exercise and social support.
Detection of the HLA-B27 allele using a commercial kit

The main topic of the presented work is the detection of the HLA-B27 allele using a commercially available kit from the company BAG HEALTH CARE. The HLA-B27 molecule has been linked to some diseases. The best known is the association with ankylosing spondylitis or Bechterew's disease. Ankylosing spondylitis is a lifelong disease, manifested by an inflammatory reaction mainly at the site of the vertebrae of the spine and large joints such as the hips, shoulders and knees. Thanks to the association, the detection of the HLA-B27 allele can be used as a molecular marker for early diagnosis, mitigation and improvement of the course of treatment of ankylosing spondylitis. The theoretical part of the thesis specifies ankylosing spondylitis and the main histocompatibility complex with emphasis on the key molecule HLA-B27. The practical part of the bachelor thesis is focused on sample collection, DNA isolation and DNA amplification. This is followed by the laboratory detection of the HLA-B27 allele itself using a commercial kit using gel electrophoresis to visualize DNA fragments. Thus, 20 anonymized DNA samples from volunteers were examined. Of the 20 samples examined, positivity for the HLA-B27 allele was demonstrated in 6 samples of volunteers who had different clinical manifestations of Bechterew's disease.
Possibilities of physiotherapy within the complex spa stay in patients with dg. Morbus Bechterev in different degrees of illness

The bachelor thesis is focused on patients diagnosed with Bechterew's disease or ankylosing spondylitis, who spent a comprehensive stay in a spa facility. The thesis takes into account the classification of Bechterew's disease into five stages, because in each stage the therapy of patients differs and is adapted to the stage of the disease. The main aim of the work is to map the possibilities of physiotherapy within a complex stay in a spa for the diagnosis of Bechterew's disease and to describe the methods which are used. According to the obtained information about the possibilities of physiotherapy, with their use, physiotherapeutic plans are compiled based on the health status of patients in various stages of the disease. Qualitative research was conducted to summarize the possibilities of physiotherapy within a spa stay and to evaluate the impact of the combination of spa care with physiotherapy.According to the results, the effect of individual physiotherapy at the end of the spa stay was evaluated as very beneficial on the patient's health. The achieved results of the research attest the undoubted importance of movement therapy in the comprehensive treatment of Bechterew's disease. Balneotherapy procedures have also been shown to improve or maintain patients' health. In addition to intensive physiotherapy, the therapeutic effect depends, for example, on the natural healing source in the respective treatment facility, but also on the change in the environment in which the patient stayed for four weeks. The calm spa environment has a beneficial effect on the patient's mental state. If physiotherapy is supplemented with special spa programs, the effectiveness of the treatment increases. The work could be used as a new source of information about the used methods and spa care, not only for students, but also for medical staff members or the non - professional public. According to the information obtained, improving the education of patients about the disease, treatment options and regimen measures could also be beneficial for the practice.

The rehabilitative treatment insists foremost upon spinal flexibility maintenance which is herewith a part of the diseas progression prophylaxis. In case of disease acute phase, I carry elimination the complications associated with pains caused by fault motional stereotypes in particular. After pains alleviation, I focus on eliminating fault motional habits.

The theoretic part describes the characteristics of AS as the main representative of spondylarthritides, including its diagnosis and testing methods. It also contains an overview of current approaches and strategies of a treatment. It focuses on biological treatment with TNF-α blockers. The practical part of the thesis illustrates the findings on the example of a patient with AS treated with biological therapy. It evaluates the subjective feelings in response to treatment and compares the results of treatment with standard therapy (i.e., without biological treatment) on another patient under otherwise similar conditions.

Title: Application of backstroke to a patient with ankylosing spondylitis Objectives: To evaluate the effectiveness of the application of backstroke with chosen parameters to a patient with ankylosing spondylitis. To compare the effectiveness of this swimming exercise programme with a traditional exercise programme held in a gymnasium. This work investigates whether it is suitable to recommend backstroke swimming to patients with ankylosing spondylitis as an effective form of physical exercise for improving range of movements and reducing pain. Methods: The effectiveness of a 9 week long swimming programme was evaluated using a range of movements of the glenohumeral and coxal joints and of the spine. The presence of pain was measured by the Bath Ankylosing Spondylitis Diasese Activity Index. The same parameters were measured for another patient with the similar activity of the disease who took part in group exercises in a gymnasium. At the end of the programmes the parameters were compared and evaluated. Results: The swimming programme has improved the patient's posture and has strengthened back muscles. Epidemiological aspects of inflammatory rheumatic diseases and diffusional diseases of binding tissue.

v anglickém jazyce Introduction: No information was known about frequency of common inflammatory disorders in rheumatology in the Czech Republic. Aims of the study: To estimate the standardized annual incidence (INC) and point prevalence (PREV) of six diseases (rheumatoid arthritis-RA, juvenile idiopathic arthritis-JIA, gout, psoriatic arthritis-PsA, ankylosing spondylitis-AS, reactive arthritis-ReA) in a population-based study in two regions of the Czech Republic (CR). Methods: INC: Incident cases were registered on condition that the definite diagnosis was confirmed according to existing classification criteria during the study period. PREV was studied on the basis of identification of established diagnoses at a time point. Crude rates were standardized for age and sex. Results: Both INC and PREV are shown per 100.000 inhabitants. RA INC:31 (95%CI 20-42), PREV:610 (95%CI 561-658). Gout-INC:41 (95%CI 28-53), PREV:300 (95% CI 266-334). JIA-INC: 13 (95% CI 1-20), PREV:140 (95%CI 117-280).
